Summary

Globally, a spotlight is being placed on difficult caesarean deliveries and particularly caesareans complicated by an impacted fetal head. When the fetal head is low and wedged in the maternal pelvis, the risk of maternal and fetal complications increase. During this hands-on workshop, we will be discussing evidence based techniques to assist with disimpaction as well as running practical drills to familiarise the participants with these techniques.

Objectives:

  • Recognise early when a second-stage caesarean section is likely to be complex, and demonstrate appropriate preparation, team communication, and choice of delivery strategy to optimise maternal and neonatal safety.
  • Safely perform and assist with key techniques for delivery of the impacted fetal head at second-stage caesarean section (including push, reverse breech extraction, and alternative manoeuvres), using correct hand placement and force vectors on simulation models.
  • Identify and minimise common maternal and neonatal complications associated with second-stage caesarean delivery, including uterine extensions, haemorrhage, and neonatal trauma, and outline immediate management steps when complications occur.
